FeLV Vaccine for Cats: Core for Every Kitten, a Lifestyle Choice After That

Core for kittens, noncore for adults
The 2020 AAHA/AAFP feline vaccination schedule puts FeLV in its core block for kittens and footnotes the exception: FeLV is noncore for cats older than one year of age.
Cornell's Feline Health Center states the same thing from the clinical side — the vaccine is now considered core for kittens because of their higher risk of developing progressive infection.
So the common shorthand, that FeLV is "the outdoor cat vaccine", is only true from a cat's first birthday onwards. For a kitten it is core, in the same block as panleukopenia and rabies.
The reason is age rather than lifestyle. Cornell notes that although any exposed cat can become infected, kittens are at greater risk than adults because of their immature immune systems, and that kittens are much likelier than adults to develop the progressive form.
Test before you vaccinate, and why
A vaccine does nothing for a cat that is already infected, which is why testing comes first — and why a kitten series that includes FeLV also includes a retroviral test on the invoice.
Cornell's testing guidance is more careful than most owners expect. Two blood tests detect the viral protein FeLV p27: an in-clinic ELISA used as a screen, and an IFA. False positives are possible on both, so confirmatory testing is always recommended after a positive screen, and PCR is the recommended follow-up.
Timing matters as well. All these tests can return a false positive with very recent infection, so where recent exposure is a concern Cornell advises repeating in 3 to 6 weeks, with most positives followed up 6 to 12 weeks later to establish whether the infection is progressive or regressive.
One myth is worth killing outright: Cornell states that FeLV vaccines do not cause false positive results on ELISA, IFA or any other available FeLV test. Vaccinating does not make future testing unreadable.
The schedule
The AAHA/AAFP table marks 8 weeks as the earliest recommended dose for FeLV and instructs practices to revaccinate 3 to 4 weeks after that first dose, with a footnote that the earliest recommended age may vary and the manufacturer's guidelines govern.
A booster follows at one year, in the same visit as the first annual rabies and, where the kitten did not have the optional six-month dose, alongside the FVRCP conversation.
After that, the table sets the interval by exposure and by product. Both the recombinant and inactivated vaccines are revaccinated annually where regular exposure to FeLV-positive cats is likely; for the inactivated product the table also allows every 2 to 3 years where such exposure is less likely.
That is an unusual instruction on a vaccine schedule, and it is the clearest statement anywhere that this vaccine is meant to be reassessed rather than repeated by default.
The adult decision is about the cat's actual life
After a cat's first birthday, the question is exposure, and Cornell defines exposure precisely: prolonged close contact or bite wounds with infected cats, cats living with infected or unknown-status cats, cats allowed outdoors unsupervised where they may be bitten, and kittens born to infected mothers.
The virus itself is fragile. Cornell records that FeLV does not survive long outside a cat's body — probably less than a few hours under normal household conditions — so infection from the environment without prolonged close contact with an infected cat is unlikely.
That fragility is what makes the indoor/outdoor line meaningful here in a way it is not for rabies. A genuinely indoor-only adult cat in a closed household has a real case for stopping FeLV boosters, and it is the guideline's own case, not a corner cut.
The judgement should be about the cat's real life rather than its intended one. A cat that escapes, a household that fosters, a new arrival of unknown status, a screened porch a neighbour's cat visits — each of those is exposure.
What FeLV does if a cat gets it
Cornell puts prevalence at 2 to 3% of all cats in the United States and Canada, rising to as much as 30% in cats that are ill or otherwise at high risk.
Outcomes after exposure are not binary. At least 20 to 30% of exposed cats mount an abortive infection and clear the virus entirely; roughly 30 to 40% develop a regressive infection where the virus is in the genome but not replicating; and roughly 30 to 40% develop a progressive infection, with kittens far likelier to land in that last group.
Progressive infection is the one with the prognosis. Cornell describes FeLV as the most common cause of cancer in cats, a cause of blood disorders, and a cause of immune deficiency that lets ordinary organisms cause severe illness — with a median survival after diagnosis of about two and a half years.
There is no cure. Cornell is explicit that the only sure way to protect cats from FeLV is to prevent exposure, and that vaccination reduces risk without protecting 100% of vaccinated cats.
Where the answer is genuinely no
A single adult indoor cat, in a household taking in no new cats, with no outdoor access and no unknown-status housemates, is the case the guidelines describe as noncore. Skipping the booster there is following the schedule rather than deviating from it.
Two things change that answer without much warning: a new cat, and a cat flap. Test before introducing any cat to the household — Cornell recommends testing all cats before introduction and housing infection-free cats separately from infected ones.
